More detail on this person: TH-55A on take off enter erratic right descending turn back to heliport at tree top level, out of control nose low dive in 90 RT BANK.

Investigation showed a one inch hole in the exhaust manifold, inside the cabin heater muff. There was no post crash fire. His CO level was such that he would have been a vegetable had he survived the impact. His was the first flight of a cool Nov. morning, probably pumped himself full of CO before the bubble cleared, and before he pulled pitch! RIP, dear Brother!
