Accident Summary:

 THE FLIGHT WAS AN EXTRACTION MISSION WHICH ORIGINATED AT COORDINATES XS 591989, AT APPROXIMATELY 1210 HOURS, 2 OCTOBER 19709 THE FLIGHT CONSISTED OF THREE AIRCRAFT, OF WHICH 64-13526 WAS THE TRAIL SHIP. THE FLIGHT WAS TO PROCEED TO FIRE SUPPORT BASE LYNCH FOR TROOP DROP OFF. THE AIRCRAFT, 64-13526, WAS APPROXIMATELY 2 TO 3 MINUTES BEHIND THE FLIGHT WHEN IT DEPARTED THE PICK-UP ZONE. AFTER CLEARING THE BARRIERS, THE AIRCRAFT COMMANDER EXPERIENCED A POWER PROBLEM AND STARTED A SHALLOW LEFT-HAND TURN IN ORDER TO RETURN TO THE PZ. AFTER BEING AIRBORNE FOR 30 TO 45 SECONDS, THE AIRCRAFT STRUCK A TREE 110 FEET HIGH, FELL AT AN ANGLE OF 60 DEGREES AND STRUCK THE GROUND 75 FEET FROM THE INITIAL POINT OF CONTACT. OTHER THAN THE RIGHT SKID TOE AND PORTIONS OF THE MAIN ROTOR BLADE, THERE WAS NO WRECKAGE DISTRIBUTION. THERE WAS A FIRE.\\
