Description: The Delta Avn Bn (later to be renamed the 13th Avn Bn) reported the entire crew of 4 US personnel and 1 ARVN observer were killed instantly during a CA approximately 18 miles south west of Bac Lieu when their armed UH-1B was shot down, crashed and burned escorting troop carriers into the LZ. The report did not identify the company, but the aircraft was from the 114th Avn Co.
