More detail on this person: CH-47A accident while serving as IP for student pilot B. ADAMS at Ft. Rucker, AL.CH-47A 61-02425 was on take-off when, at about 200' AGL, the aft blades went to full pitch which caused the aft pylon to climb, flipping the A/C over to an inverted position on Ft. Rucker.
