Accident Summary:

 AIRCRAFT ^69-15119^ WAS PARTICIPATING IN A FINAL EXTRACTION OF FSB MO. THE FLIGHT HAD LOADED ITS TROOPS ON THE NORTHERN LOG PAD AT FSB MO. THE FIRST AIRCRAFT LIFTED ON A HEADING OF 270, CLEARED THE BARRIERS AND PROCEEDED TO CLIMB OUT. ^119^ INITIATED ITS TAKE OFF ON THE SAME HEADING AND STARTED TO CLEAR THE BARRIERS. WHILE ATTEMPTING TO CLEAR THE BARRIERS THE AIRCRAFT HAD SOME MINOR BLADE STRIKES ON BAMBOO. THE AIRCRAFT THEN STARTED A LEFT BANK TO AVOID HITTING A LARGE, DEAD TREE. THE CORRECTIVE ACTION WAS LATE AND THE MAIN ROTOR SYSTEM STRUCK THE TREE ON THE RIGHT SIDE OF THE AIRCRAFT. THE AIRCRAFT STARTED SETTLING TOWARD THE GROUND INTO A CLEARING IN THE DIRECTION OF FLIGHT. THE AIRCRAFT SETTLED TAIL LOW ON ITS RIGHTSIDE AND BURST INTO FLAMES UPON STRIKING THE GROUND. THE AIRCRAFT WAS A TOTAL LOSS. THREE CREW MEMBERS AND TWO PASSENGERS WERE TRAPPED IN THE WRECKAGE AND WERE BURNED. ONE CREW MEMBER AND THREE PASSENGERS WERE EVACUATED TO MEDICAL FACILITIES.\\
