Accident Summary:

 THE CREWS OF UH-1H, SN: 68-16123 AND SN: 68-15648 WERE ON A FLIGHT FROM BIEN HOA, RVN TO FSB GREEN, RVN. THE AIRCRAFT WERE AT 4000 FEET VFR ON TOP. THE FLIGHT LEADER (#123) CALLED FOR A FORMATION SPREAD OF A MINIMUM OF 5 ROTOR DISKS TO PERFORM DER CHECKS. (FORMATION: VEE OF 5) WHILE PERFORMING THE DER CHECKS AIRCRAFT #648 ACCELERATED FORWARD COMING UP ALONG THE LEFT REAR SIDE OF AIRCRAFT #123. AT THE SAME TIME AIRCRAFT #648 DRIFTED RIGHT COLLIDING WITH AIRCRAFT #123 IN THE TAIL BOOM AREA. THE TAIL BOOM FROM #123 WAS SEVERED. AS THE ROTOR SYSTEM SEPARATED, THE AIRCRAFT INVERTED AND DROPPED OUT OF SIGHT. AIRCRAFT #648 PITCHED UP AND LEFT. THE ROTOR SYSTEM STARTED TO SEPARATE AND THE AIRCRAFT DROPPED OUT OF SIGHT IN A NOSE HIGH ATTITUDE. THE AIRCRAFT FELL FROM 4000 FEET AND IMPACTED APPROXIMAATELY 250 METERS APART.\\
