Description: The 155th Aviation Company unit history states that material failure of a transmission drive shaft caused one of the company's aircraft to crash as it was returning to home base from a combat assault. The aircraft commander subsequently died of injuries sustained in the accident. The pilot was evacuated to a hospital in the States. The two other crew members received only minor injuries.
