Accident Summary:
AIRCRAFT WAS ASSIGNED A MISSION TO HAUL 6 PASSENGERS AND A WATER TRAILER FROM ^PHUOC VINH TO FIRE SUPPORT BASE DOT^. THE WATER TRAILER WEIGHING 6500 LBS WAS LOADED BY EXTERNAL CARGO SLING. THE AIRCRAFT ARRIVED AT ^FIRE SUPPORT BASE DOT^ AT APPROXIMATELY ^1045 HOURS 26 NOVEMBER 1968^ AND INITIATED A CIRCLING APPROACH TO THE OFF LOADING PAD AT ^FIRE SUPPORT BASE DOT^. THE FINAL APPROACH WAS ON A GENERAL EASTERLY HEADING. THE PILOT, ^CW WALLACE^ WAS AT THE CONTROLS AND THE CREW CHIEF WAS STATIONED AT THE CARGO HOOK ACCESS DOOR ON THE FLOOR OF THE CARGO COMPARTMENT OBSERVING THE SLING LOAD WATER TRAILER. AT ABOUT 100 FEET OFF THE GROUND ^SP HOLLEY^ CALLED OUT OVER THE INTERCOM THAT THE LOAD WAS 100 FEET OFF THE GROUND. AT THIS TIME THE #1 ENGINE BLEED BANDS STARTED POPPING. THE AIRCRAFT COMMANDER CHECKED THE N1 PERCENT INDICATORS AND FOUND THAT THE N1 PERCENT ON #1 ENGINE HAD DROPPED TO ZERO. AT THIS TIME HE ORDERED THE SLING LOAD TO BE RELEASED AND TOOK CONTROL OF THE AIRCRAFT. THE WATER TRAILER DROPPED ABOUT 25 TO 50 FEET NEAR THE INTENDED LOADING SPOT. SINCE THERE WERE GROUND TROOPS IN THE IMMEDIATE AREA AND NO GOOD LANDING SPOT READILY AVAILABLE,^CW HOLLEN^ELECTED TO GO AROUND ON THE #2 ENGINE WITH THE INTENTION OF EITHER CIRCLING BACK AROUND TO A GOOD LANDING AREA OR FLYING TO A NEARBY AIRFIELD. AS HE PROCEEDED AWAY FROM THE DROP OFF PAD TOWARDS THE SOUTH, THE ROTOR RPM DECAYED TO ABOUT 190-200 RPM AND HELD AT THAT POINT. THROUGH RAISING AND LOWERING THE THRUST LEVER, THE AIRSPEED INCREASED TO 50 KNOTS AND THE AIRCRAFT STARTED TO CLIMB. AT THIS POINT ^CW HOLLEN^ STARTED LOWERING THE THRUST LEVER IN ORDER TO BUILD THE RPM BACK UP TO NORMAL OPERATING RANGE AT WHICH POINT THE #2 ENGINE STARTED HAVING BLEED BAND POPPING. PERSONNEL ON THE GROUND STATED THAT FLAMES WERE COMING OUT OF THE ENGINE AND THAT THE ENGINE MADE POPPING NOISES. AT THIS TIME ^CW HOLLEN^ INITIATED A TURN TO THE EAST TO CIRCLE BACK TOWARDS ^FIRE SUPPORT BASE DOT^ WHEN HE NOTICED A LOSS OF POWER. A CHECK OF THE #2 ENGINE N1 GAGE REVEALED THAT THE N1 SPEED HAD DROPPED TO 40%. ^CW HOLLEN^ STATED THAT HE KNEW AT THIS TIME THAT HE MUST EXECUTE AN EMERGENCY LANDING. THERE WAS A PARTIALLY CLEARED AREA TO HIS LEFT FRONT WHICH HE SELECTED. THE AIRCRAFT HIT IN A LEVEL, NOSE HIGH ATTITUDE, ON THE REAR GEAR. HE STATED THAT THE TOUCHDOWN WAS NOT VERY HARD, BUT WHEN THE NOSE CAME DOWN, THE FORWARD ROTOR BLADES STRUCK THE SURROUNDING TREES CAUSING THE AIRCRAFT TO ROLL OVER ON THE RIGHT SIDE. A TREE STUMP ENTERED THE COCKPIT ON THE PILOT'S SIDE CAUSING A MINOR ABRASION ON ^CW WALLACE'S^ LEFT LEG. THE ENGINE REMAINED RUNNING AT 40% N1 FOR 2-3 MINUTES AFTER THE CRASH, AT WHICH TIME ^CW WALLACE^ REENTERED THE AIRCRAFT TO SHUT OFF THE SWITCHES.\\
